I have not tried it on Thonny yet but I have noticed with Pycharm the Raspberry Pico is not doing a soft reboot. I have to unplug the USB and plug it back in to run the program. I have followed the steps provided on this fantastic step-by-step video (see blow link). I will try Thonny on a Mac later and tell you if it works or not.

Instead of plug off and replug in.... may be could you try to set the "run" pin to the ground.

I picked up one of these https://shop.pimoroni.com/products/pico-omnibus and a couple of these https://shop.pimoroni.com/products/pico-proto to use with my Pico.
A couple of buttons and a 4-way I2C connector later . . .

Maybe you want try ctrl-d in terminal/REPL, instead of unplug and replug the USB?
